COMPLETE YOUR FORM
Fill out the designated side with all required information.
On SIDE 2, complete first name, last name, and birthday only.
Circle the part of the program you are trying out for: ELITE, PREP, EXHIBITION (6 & UNDER), or CHEERABILITIES.
CHEERABILITIES is available for athletes of all abilities. Additional details will be provided prior to placements.

Sign up through the EVENTS CALENDAR in the PARENT PORTAL or on our APP. (App download links are available at the bottom of this page.)
To stay competitive in all-star cheerleading, athletes (except LEVEL 1, EXHIBITION, and CHEERABILITIES) must have all level-appropriate skills, plus two ADVANCED and one ELITE skill. If you are an EXHIBITION or CHEERABILITIES athlete, you only need to select the LEVEL 1 form.
